Admit card of NEET-UG-2020 (Downloaded) – Self Attested 5. Mark-sheet of NEET-UG-2020 examination (Downloaded) – Self Attested 6. Nationality Certificate issued by District Magistrate/Addl. District Magistrate or Metropolitan Magistrate (Competent Authority for issue of such certificate)/Valid Indian passport/School Leaving Certificate of H.S.S.C./12th Std. indicating the nationality of the candidate as ‘Indian’. 7. Domicile Certificate of Candidates from Maharashtra State issued by District Magistrate / Metropolitan Magistrate/ Addl. District Magistrate or Competent Authority for issue of such certificate. 8. H.S.S.C. (or equivalent) examination mark-sheet. 9. S.S.C. (or equivalent) passing certificate 10. H.S.S.C. School Leaving Certificate 11. Migration Certificate issued by the respective Board / University applicable to: a) CBSC & ICSC Board b) After passing H.S.S.C./Qualifying examination, students who have joined any other course for period of more than six months 12. Gap Affidavit on Rs. 100/- stamp paper by the student duly certified by Executive Magistrate with ‘Annexure A’ as per Govt. Resolution dated 09 March 2015 (if Applicable) 13. Medical Fitness Certificate on Letter Head as per preference brochure (Annexure- H) 14. Proof of permanent residential address (Xerox – Aadhar card / Passport / BSNL telephone bill / Electricity bill)- self attested 15. Parent’s Pan Card (Xerox) – self attested 16. Caste Certificate 17. Caste Validity Certificate 18. Non Creamy Layer Certificate valid up to 31/03/2021 (VJ, NT1, NT2, NT3, SEBC & OBC including SBC). 19 Income Certificate (Original) mentioning total income of family issued by Tahasildar (Basic Pay + DA + Other Allowances) ? If Parent is Farmer, latest 7/12 certificate of Talathi (Xerox) ? If Parents are in Government/Private Service, Form No. 16 of 2019-20 (Xerox). ? If Parents are Businessman, ITR of 2019-20 (Xerox) ?
